This was meant to be his impersonation of women competitive weightlifters. He imitates an overhead press while grunting and moaning. He sticks his tongue out because he thinks that is what women do when weightlifting?
He has to know what this looks like, right? He has to be aware that he’s doing a double jerk-off motion while moaning. He worked in Hollywood for decades, he is a performer, he knows what it looks like.
Prior to doing this bit, he prefaces by saying that his wife told him not to do it and that why he’s going to do it. This is a classic boomer joke, hating your wife. Trump says "My wife hates when I do this. She said ‘it is so unpresidential’. I said ‘well I did become president.’ "
